Referring Physician Access to OCSRI Physician Portal

In order for referring physicians to access secure OCSRI Patient Information through a dedicated Physician Portal, follow these steps:

1. Fill out the “Non-Employee Confidentiality Agreement/EMR Access Request” below and submit to OCSRI’s IT Department as indicated on the form.
2. A user account will be established for the OCSRI Physician Portal and the Physician will be notified of further access information.
3. Use the “Using the OCSRI Physician Portal” document below to guide access to the portal.
